Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 5 years hospital case management experience required License: Current license required Certifications: Not required Must-Have: - Experience with Cerner/Power Chart - Knowledge of Wellsky (formerly naviHealth) - Familiarity with PASRR entry (ASCEND) - Knowledge of PAE Description: Cerner experience is required. Ratios are 1:25 to 1:28. First-time travelers are accepted and will be reviewed. No weekend requirements are necessary. Clinician may float within scope to meet facility needs. On-call requirement is applicable. Experience with case management is essential. Interdisciplinary support includes various services. The patient age groups include adults and geriatrics. Daily census is approximately 575. Number of beds is 710. Candidate should be open to working some holidays. All requests for time off must be discussed during the interview. Compliance documents are required prior to the start date.
