Position Title: Clinical Resource Nurse Department: Hospital Medicine Reports to Title: FMD JOB DESCRIPTION OVERVIEW: This position provides leadership to local operations of the Hospital Medicine program including achievement of the program's goals, providing quality care, establishing customer service relationship with the hospital and community, including client service support for contracted and employed practitioners. The position is located onsite at Holston Valley Medical Center. ESSENTIAL DUTIES AND RESPONSIBILITIES:. . Functions together with the Facility Medical Director (FMD) as a team to manage client based clinical relations. . Works directly with case management and providers to provide clinical support to assist with discharge planning and patient relations. . Works with the providers and FMD to act as a liaison with hospital staff including but not limited to nursing, quality, case management and administration. . Works proactively to anticipate and manage day to day operational and clinical challenges promoting strongly positive client relationships. . Performs other special projects or additional responsibilities as may be directed. . Coordinate with PCPs to set up appropriate appointment at time of discharge. . Follow up with patient post discharge to ensure medication supply and secure feedback on survey forms. QUALIFICATIONS/EXPERIENCE: . Position requires education level of licensed practicing nurse (LPN) or registered nurse (RN) with minimum three years clinical experience. . Strong/advanced computer skills should include at minimum: Word, Excel and PowerPoint. Proficiency with spreadsheets, tables and graphs is required. . Ability to communicate effectively (written and oral). . Demonstrates good customer service skills and ability to use tact and diplomacy in dealing with internal and external customers. . Demonstrates ability to positively handle confidential information appropriately, and maintain office workflow systems. . Willing to work occasional evenings and/or weekends attending physician programs as deemed. . Performs other duties and responsibilities as assigned as contractually obligated with the client. PHYSICAL/ENVIRONMENTAL DEMANDS: . Job is performed in well-lighted modern office setting; . Occasional lifting/carrying (10 pounds or less); . Moderate to high stress levels; . Prolonged work hours (evening and weekends); This position may require manual dexterity and/or frequent use of the computer, telephone, office machines (copier, scanner, fax) and/or the ability to perform repetitive motions and/or meet production standards to comply with the essential functions. Also, may require physical and/or mental stamina to work overtime, additional hours beyond a regular schedule and/or more than five days per week.
