RN -Nurse Navigator (FT) 8a-5p Corp Navigators - Kingsport, TNJob ID: 004FMEBALLAD CORPORATENursing - Corporate NavigatorsFull-time (scheduled 72 hrs or more per pay period)Job DescriptionSCOPE OF POSITIONThe Nurse Navigator is responsible for the coordination of diagnostic, treatment and support services. They serve as an educator, care coordinator, consultant, patient advocate, counselor, and collaborator for cancer and/or other chronically ill patients. The role is instrumental in developing and implementing changes to support a holistic and coordinated approach to cancer care services and other services related to chronic illnesses by ensuring a smooth flow of patient information and care. The position is a liaison between the providers, professional health care staff, and other involved parties. The position serves on boards and committees relating to cancer care and/or other chronic illnesses. The Nurse Navigator serves as community spokesperson for coordinated care together with the medical community. The position facilitates interdisciplinary communication within the care continuum to promote continuity of care. The position coordinates multidisciplinary care meetings related to cancer and/or chronically ill patients and communicates treatment decisions.REPORTING RELATIONSHIPSee Table of Organization.EDUCATION AND EXPERIENCEAssociate's or Bachelor's degree from an accredited Nursing program. Oncology experience is preferred. Pediatric experience is preferred for a nurse navigator working with pediatric patients. Must become certified in national oncology certification within 1st year.Valid and active RN licensure in appropriate stateCPR requiredChemotherapy Biotherapy Certification (ONS/ONCC) or Oncology Certified Nurse (OCN) preferredNational certification in oncology (adult, pediatric, or other population specific) from Oncology Nurse Certification Corporation (ONCC) within 1st year requiredAny additional specific oncology based certifications must be evidenced based nationally accredited nursing certificationsRequirementsNo additional requirements from any stated in the above description.by Jobble
